Assistance Application

WE ARE CURRENTLY ONLY ACCEPTING APPLICATIONS FOR SPAY/NEUTER PROCEDURES AND PROCEDURES MUST BE PERFORMED AT THE HSOP SPOT CLINIC IN PARKERSBURG WV.

If you are requesting financial assistance for cats or dogs, and the animal is not already spayed or neutered, we require the spaying or neutering of the animal unless the veterinarian states it is not medically prudent.  Please be aware that accepting assistance commits you to supporting the health of our animal community by agreeing to the spaying or neutering of your animal.  If needed, we will include the cost of the procedure in our evaluation of your assistance request.

You must be willing to give individuals associated with Hearts & Hands 4 Paws authority to speak with your veterinarian or other service provider to discuss and authorize treatment, and to make payment arrangements.

All payments are made directly to your veterinarian or other service provider.  Funding is not provided directly to pet owners or for services already rendered.

All assistance is based on availability of funds at the time your request is received.

I understand that Hearts and Hands 4 Paws will contact my veterinarian or service provider for information, to arrange payment, and to request documentation to support the payment.

I also understand that if my pet is a dog or cat, and is not already spayed or neutered, I will be required to agree to spay or neuter the animal unless a veterinarian says the surgery is not medically prudent.
